Aftercare planning begins well before discharge and is integrated throughout treatment. Clients work collaboratively with their care team to identify next steps and ongoing supports aligned with their goals.
Aftercare planning may include referrals to outpatient services, continued individual or group therapy, MAT coordination when applicable, recovery support group recommendations, housing or vocational support connections, relapse prevention planning, and coordination with community resources.
Continuing care services are designed to promote sustained engagement in recovery. Clients may continue participating in outpatient services, receive periodic check-ins, or engage in recommended support systems based on clinical need.
This ongoing connection helps reinforce accountability, encourage healthy routines, and provide support during periods of transition or increased stress.
